A child in a clinic somewhere does not get a vaccine this month.
The reason is written nowhere she will ever read. It is on a spreadsheet in a finance ministry, in a line that has to be paid before hers - the line marked interest. The medicine, the nurse's wage, the new roof for the school: these can wait, be trimmed, be struck out. The interest cannot. It falls due on a fixed day, and if it is not paid the country's own borrowing costs rise the next morning, and next year the same line is larger still.
So the vaccine is the thing that gets cut.
Multiply that clinic by a continent. By the count of one UN agency this July, roughly forty-eight developing countries now spend more on the interest on their debts than on the health of their people, or on their schools. A World Bank study this spring found servicing already outweighs health spending in four of five African countries, and called the result, plainly, a development shutdown. Three quarters of the world's low- and middle-income countries are in debt distress or close to it. And more than three and a half trillion dollars of poorer countries' borrowing falls due between now and 2028 - to be repaid, or borrowed again, at rates far above those at which it was first taken.

How it always begins
It begins in the good years. Money is cheap, lenders are eager, and a government borrows - to build, sometimes to survive, sometimes to line a few pockets, but mostly for the ordinary reasons the poor borrow. Then the weather turns. Rates rise in distant capitals; a currency falls; a harvest fails. The debt that was easy to carry becomes heavy, and because much of it was short - due again within a few years - it must be re-borrowed at the new, punishing rate. What was a loan becomes a treadmill.
The world has watched this exact sequence before. In 1982 Mexico announced it could no longer pay, and most of Latin America went down with it into what the region still calls the lost decade - a quarter-century before poverty returned to where it started. Relief came eventually: the restructurings of the late 1980s, then, at the turn of the millennium, a campaign that persuaded the wealthy world to cancel the poorest countries' debts outright. The treadmill slowed. Then the cheap years came round again, and the borrowing came round again, and the pandemic forced even the prudent to borrow to keep their people alive, and in 2022 the rates rose - and here we are, at the next turning of the same wheel.
Notice what the wheel runs on. Not, mostly, villainy. It runs on a mechanism so normal we have stopped seeing it as a choice: that money lent should come back larger, and larger the longer the borrower struggles - that the very difficulty of the one who cannot pay is what makes his debt grow.
The Qur'an saw that mechanism. It gave it a name, and did not bless it.
The name
The name is ribā - the increase. The growth added onto a loan for no reason but the passage of time and the weakness of the borrower. And the Qur'an's verdict on it is among the sternest it passes on any transaction:
"Those who swallow usury will not rise, except as someone driven mad by Satan's touch... But Allah has permitted commerce, and has forbidden usury."
الَّذِينَ يَأْكُلُونَ الرِّبَا لَا يَقُومُونَ إِلَّا كَمَا يَقُومُ الَّذِي يَتَخَبَّطُهُ الشَّيْطَانُ مِنَ الْمَسِّ ... وَأَحَلَّ اللَّهُ الْبَيْعَ وَحَرَّمَ الرِّبَا

ٱلَّذِينَ يُنفِقُونَ أَمْوَٰلَهُم بِٱلَّيْلِ وَٱلنَّهَارِ سِرًّۭا وَعَلَانِيَةًۭ فَلَهُمْ أَجْرُهُمْ عِندَ رَبِّهِمْ وَلَا خَوْفٌ عَلَيْهِمْ وَلَا هُمْ يَحْزَنُونَ274 ٱلَّذِينَ يَأْكُلُونَ ٱلرِّبَوٰا۟ لَا يَقُومُونَ إِلَّا كَمَا يَقُومُ ٱلَّذِى يَتَخَبَّطُهُ ٱلشَّيْطَٰنُ مِنَ ٱلْمَسِّ ۚ ذَٰلِكَ بِأَنَّهُمْ قَالُوٓا۟ إِنَّمَا ٱلْبَيْعُ مِثْلُ ٱلرِّبَوٰا۟ ۗ وَأَحَلَّ ٱللَّهُ ٱلْبَيْعَ وَحَرَّمَ ٱلرِّبَوٰا۟ ۚ فَمَن جَآءَهُۥ مَوْعِظَةٌۭ مِّن رَّبِّهِۦ فَٱنتَهَىٰ فَلَهُۥ مَا سَلَفَ وَأَمْرُهُۥٓ إِلَى ٱللَّهِ ۖ وَمَنْ عَادَ فَأُو۟لَٰٓئِكَ أَصْحَٰبُ ٱلنَّارِ ۖ هُمْ فِيهَا خَٰلِدُونَ275 يَمْحَقُ ٱللَّهُ ٱلرِّبَوٰا۟ وَيُرْبِى ٱلصَّدَقَٰتِ ۗ وَٱللَّهُ لَا يُحِبُّ كُلَّ كَفَّارٍ أَثِيمٍ276 إِنَّ ٱلَّذِينَ ءَامَنُوا۟ وَعَمِلُوا۟ ٱلصَّٰلِحَٰتِ وَأَقَامُوا۟ ٱلصَّلَوٰةَ وَءَاتَوُا۟ ٱلزَّكَوٰةَ لَهُمْ أَجْرُهُمْ عِندَ رَبِّهِمْ وَلَا خَوْفٌ عَلَيْهِمْ وَلَا هُمْ يَحْزَنُونَ277 يَٰٓأَيُّهَا ٱلَّذِينَ ءَامَنُوا۟ ٱتَّقُوا۟ ٱللَّهَ وَذَرُوا۟ مَا بَقِىَ مِنَ ٱلرِّبَوٰٓا۟ إِن كُنتُم مُّؤْمِنِينَ278 فَإِن لَّمْ تَفْعَلُوا۟ فَأْذَنُوا۟ بِحَرْبٍۢ مِّنَ ٱللَّهِ وَرَسُولِهِۦ ۖ وَإِن تُبْتُمْ فَلَكُمْ رُءُوسُ أَمْوَٰلِكُمْ لَا تَظْلِمُونَ وَلَا تُظْلَمُونَ279 وَإِن كَانَ ذُو عُسْرَةٍۢ فَنَظِرَةٌ إِلَىٰ مَيْسَرَةٍۢ ۚ وَأَن تَصَدَّقُوا۟ خَيْرٌۭ لَّكُمْ ۖ إِن كُنتُمْ تَعْلَمُونَ280 وَٱتَّقُوا۟ يَوْمًۭا تُرْجَعُونَ فِيهِ إِلَى ٱللَّهِ ۖ ثُمَّ تُوَفَّىٰ كُلُّ نَفْسٍۢ مَّا كَسَبَتْ وَهُمْ لَا يُظْلَمُونَ281
274Those who spend their wealth by night and day, privately and publicly, will receive their reward from their Lord. They have nothing to fear, nor shall they grieve. 275Those who swallow usury will not rise, except as someone driven mad by Satan's touch. That is because they say, “Commerce is like usury.” But Allah has permitted commerce, and has forbidden usury. Whoever, on receiving advice from his Lord, refrains, may keep his past earnings, and his case rests with Allah. But whoever resumes—these are the dwellers of the Fire, wherein they will abide forever. 276Allah condemns usury, and He blesses charities. Allah does not love any sinful ingrate. 277Those who believe, and do good deeds, and pray regularly, and give charity—they will have their reward with their Lord; they will have no fear, nor shall they grieve. 278O you who believe! Fear Allah, and forgo what remains of usury, if you are believers. 279If you do not, then take notice of a war by Allah and His Messenger. But if you repent, you may keep your capital, neither wronging, nor being wronged. 280But if he is in hardship, then deferment until a time of ease. But to remit it as charity is better for you, if you only knew. 281And guard yourselves against a Day when you will be returned to Allah; then each soul will be rewarded fully for what it has earned, and they will not be wronged.
Read the image before the ruling. The one who consumes the increase is not described as rich, or shrewd, or comfortable. He is described as staggering - rising like a man the world spins around, a man who has lost the thread. And the verse draws a line the modern economy is built to blur: trade - value exchanged for value, risk shared between two parties - is permitted, even honoured. The increase demanded from a debtor simply because he owes and because he is late is something else, and it is forbidden. Note the verb the Qur'an chooses, twice: yaʾkulūn. They eat it. The increase does not feed the one who takes it. It is the other way round.
Nor is this a small or marginal prohibition, a footnote for the scrupulous. It is repeated, and sharpened, until it names the exact motion of a debt that compounds:
"You who believe, do not consume usury, compounded over and over."
يَا أَيُّهَا الَّذِينَ آمَنُوا لَا تَأْكُلُوا الرِّبَا أَضْعَافًا مُّضَاعَفَةً
lā taʾkulū r-ribā aḍʿāfan muḍāʿafah

يَٰٓأَيُّهَا ٱلَّذِينَ ءَامَنُوا۟ لَا تَأْكُلُوا۟ ٱلرِّبَوٰٓا۟ أَضْعَٰفًۭا مُّضَٰعَفَةًۭ ۖ وَٱتَّقُوا۟ ٱللَّهَ لَعَلَّكُمْ تُفْلِحُونَ130 وَٱتَّقُوا۟ ٱلنَّارَ ٱلَّتِىٓ أُعِدَّتْ لِلْكَٰفِرِينَ131 وَأَطِيعُوا۟ ٱللَّهَ وَٱلرَّسُولَ لَعَلَّكُمْ تُرْحَمُونَ132 ۞ وَسَارِعُوٓا۟ إِلَىٰ مَغْفِرَةٍۢ مِّن رَّبِّكُمْ وَجَنَّةٍ عَرْضُهَا ٱلسَّمَٰوَٰتُ وَٱلْأَرْضُ أُعِدَّتْ لِلْمُتَّقِينَ133 ٱلَّذِينَ يُنفِقُونَ فِى ٱلسَّرَّآءِ وَٱلضَّرَّآءِ وَٱلْكَٰظِمِينَ ٱلْغَيْظَ وَٱلْعَافِينَ عَنِ ٱلنَّاسِ ۗ وَٱللَّهُ يُحِبُّ ٱلْمُحْسِنِينَ134 وَٱلَّذِينَ إِذَا فَعَلُوا۟ فَٰحِشَةً أَوْ ظَلَمُوٓا۟ أَنفُسَهُمْ ذَكَرُوا۟ ٱللَّهَ فَٱسْتَغْفَرُوا۟ لِذُنُوبِهِمْ وَمَن يَغْفِرُ ٱلذُّنُوبَ إِلَّا ٱللَّهُ وَلَمْ يُصِرُّوا۟ عَلَىٰ مَا فَعَلُوا۟ وَهُمْ يَعْلَمُونَ135 أُو۟لَٰٓئِكَ جَزَآؤُهُم مَّغْفِرَةٌۭ مِّن رَّبِّهِمْ وَجَنَّٰتٌۭ تَجْرِى مِن تَحْتِهَا ٱلْأَنْهَٰرُ خَٰلِدِينَ فِيهَا ۚ وَنِعْمَ أَجْرُ ٱلْعَٰمِلِينَ136
130O you who believe! Do not feed on usury, compounded over and over, and fear Allah, so that you may prosper. 131And guard yourselves against the Fire that is prepared for the disbelievers. 132And obey Allah and the Messenger, that you may obtain mercy. 133And race towards forgiveness from your Lord, and a Garden as wide as the heavens and the earth, prepared for the righteous. 134Those who give in prosperity and adversity, and those who restrain anger, and those who forgive people. Allah loves the doers of good. 135And those who, when they commit an indecency or wrong themselves, remember Allah and ask forgiveness for their sins—and who forgives sins except Allah? And they do not persist in their wrongdoing while they know. 136Those—their reward is forgiveness from their Lord, and gardens beneath which rivers flow, abiding therein forever. How excellent is the reward of the workers.
Aḍʿāfan muḍāʿafah. Doubled and redoubled. Fourteen centuries before a spreadsheet could compound a balance to the second decimal, the word for it is already here - the increase that breeds increase, the sum that grows while the debtor sleeps.
What the creditor may keep
It would be easy to stop there and make this a sermon against lenders. The Qur'an does not stop there, and its next move is stranger and more searching than a condemnation. Having forbidden the increase, it turns to the creditor and tells him precisely what he is still owed:
"You shall have your capital if you repent, and without suffering loss or causing others to suffer loss."
وَإِن تُبْتُمْ فَلَكُمْ رُءُوسُ أَمْوَالِكُمْ لَا تَظْلِمُونَ وَلَا تُظْلَمُونَ

The lender is not robbed. His capital - ruʾūs amwālikum, the very heads of your wealth - is his to reclaim in full. What he may not do is take more than he gave from a man who is drowning. The verse fixes the matter on two verbs held level: lā taẓlimūna wa lā tuẓlamūn - do no wrong, and be done no wrong. The one who lent has a claim; the one who owes has a limit past which the claim becomes a wound. The increase is the crossing of that line.
And then - only now, with the ground laid, the definition made, the creditor's true due named - the Qur'an says the thing this whole essay has been walking toward. What do you do with the debtor who genuinely cannot pay?
"If the debtor is in hardship, then delay things until matters become easier for him; still, if you were to write it off as an act of charity, that would be better for you, if only you knew."
وَإِن كَانَ ذُو عُسْرَةٍ فَنَظِرَةٌ إِلَىٰ مَيْسَرَةٍ ۚ وَأَن تَصَدَّقُوا خَيْرٌ لَّكُمْ ۖ إِن كُنتُمْ تَعْلَمُونَ
wa in kāna dhū ʿusratin fa-naẓiratun ilā maysarah

Ibn Kathīr (d. 1373 CE), reading this verse, sets it against the practice of the age it came into. The people before Islam, he writes, would say to the debtor whose loan had come due: either you pay, or you increase it. Pay now, or watch the sum grow. That formula is not a relic. It is the machine described at the top of this essay - the treadmill under three quarters of the developing world, the reason a country re-borrows at a worse rate the moment it stumbles. The Qur'an met that formula and reversed it. Where the world said if he is in hardship, increase it, revelation said: if he is in hardship, wait. And more than wait - if you can, release him, and count the release not as a loss but as charity, which is better for you.
In the world's accounting, the debtor's hardship is the lender's gain - the worse his position, the more can be extracted. In the Qur'an's, it is a door - the worse his position, the more his patience is owed to him. The increase feeds on weakness. The command feeds weakness instead.
What actually grows
For the reversal is the whole point. The Qur'an does not merely forbid the increase; it calls the increase an illusion - the thing everyone chases does not, in the only ledger that finally settles, grow at all:
"God blights usury, but blesses charitable deeds with multiple increase."
يَمْحَقُ اللَّهُ الرِّبَا وَيُرْبِي الصَّدَقَاتِ
yamḥaqu llāhu r-ribā wa yurbī ṣ-ṣadaqāt

ٱلَّذِينَ يُنفِقُونَ أَمْوَٰلَهُم بِٱلَّيْلِ وَٱلنَّهَارِ سِرًّۭا وَعَلَانِيَةًۭ فَلَهُمْ أَجْرُهُمْ عِندَ رَبِّهِمْ وَلَا خَوْفٌ عَلَيْهِمْ وَلَا هُمْ يَحْزَنُونَ274 ٱلَّذِينَ يَأْكُلُونَ ٱلرِّبَوٰا۟ لَا يَقُومُونَ إِلَّا كَمَا يَقُومُ ٱلَّذِى يَتَخَبَّطُهُ ٱلشَّيْطَٰنُ مِنَ ٱلْمَسِّ ۚ ذَٰلِكَ بِأَنَّهُمْ قَالُوٓا۟ إِنَّمَا ٱلْبَيْعُ مِثْلُ ٱلرِّبَوٰا۟ ۗ وَأَحَلَّ ٱللَّهُ ٱلْبَيْعَ وَحَرَّمَ ٱلرِّبَوٰا۟ ۚ فَمَن جَآءَهُۥ مَوْعِظَةٌۭ مِّن رَّبِّهِۦ فَٱنتَهَىٰ فَلَهُۥ مَا سَلَفَ وَأَمْرُهُۥٓ إِلَى ٱللَّهِ ۖ وَمَنْ عَادَ فَأُو۟لَٰٓئِكَ أَصْحَٰبُ ٱلنَّارِ ۖ هُمْ فِيهَا خَٰلِدُونَ275 يَمْحَقُ ٱللَّهُ ٱلرِّبَوٰا۟ وَيُرْبِى ٱلصَّدَقَٰتِ ۗ وَٱللَّهُ لَا يُحِبُّ كُلَّ كَفَّارٍ أَثِيمٍ276 إِنَّ ٱلَّذِينَ ءَامَنُوا۟ وَعَمِلُوا۟ ٱلصَّٰلِحَٰتِ وَأَقَامُوا۟ ٱلصَّلَوٰةَ وَءَاتَوُا۟ ٱلزَّكَوٰةَ لَهُمْ أَجْرُهُمْ عِندَ رَبِّهِمْ وَلَا خَوْفٌ عَلَيْهِمْ وَلَا هُمْ يَحْزَنُونَ277 يَٰٓأَيُّهَا ٱلَّذِينَ ءَامَنُوا۟ ٱتَّقُوا۟ ٱللَّهَ وَذَرُوا۟ مَا بَقِىَ مِنَ ٱلرِّبَوٰٓا۟ إِن كُنتُم مُّؤْمِنِينَ278 فَإِن لَّمْ تَفْعَلُوا۟ فَأْذَنُوا۟ بِحَرْبٍۢ مِّنَ ٱللَّهِ وَرَسُولِهِۦ ۖ وَإِن تُبْتُمْ فَلَكُمْ رُءُوسُ أَمْوَٰلِكُمْ لَا تَظْلِمُونَ وَلَا تُظْلَمُونَ279 وَإِن كَانَ ذُو عُسْرَةٍۢ فَنَظِرَةٌ إِلَىٰ مَيْسَرَةٍۢ ۚ وَأَن تَصَدَّقُوا۟ خَيْرٌۭ لَّكُمْ ۖ إِن كُنتُمْ تَعْلَمُونَ280 وَٱتَّقُوا۟ يَوْمًۭا تُرْجَعُونَ فِيهِ إِلَى ٱللَّهِ ۖ ثُمَّ تُوَفَّىٰ كُلُّ نَفْسٍۢ مَّا كَسَبَتْ وَهُمْ لَا يُظْلَمُونَ281
274Those who spend their wealth by night and day, privately and publicly, will receive their reward from their Lord. They have nothing to fear, nor shall they grieve. 275Those who swallow usury will not rise, except as someone driven mad by Satan's touch. That is because they say, “Commerce is like usury.” But Allah has permitted commerce, and has forbidden usury. Whoever, on receiving advice from his Lord, refrains, may keep his past earnings, and his case rests with Allah. But whoever resumes—these are the dwellers of the Fire, wherein they will abide forever. 276Allah condemns usury, and He blesses charities. Allah does not love any sinful ingrate. 277Those who believe, and do good deeds, and pray regularly, and give charity—they will have their reward with their Lord; they will have no fear, nor shall they grieve. 278O you who believe! Fear Allah, and forgo what remains of usury, if you are believers. 279If you do not, then take notice of a war by Allah and His Messenger. But if you repent, you may keep your capital, neither wronging, nor being wronged. 280But if he is in hardship, then deferment until a time of ease. But to remit it as charity is better for you, if you only knew. 281And guard yourselves against a Day when you will be returned to Allah; then each soul will be rewarded fully for what it has earned, and they will not be wronged.
The two verbs come from opposite worlds. Yamḥaq is what happens to the moon as it wanes to nothing - a slow erasure of light. Yurbī is growth, the very root of ribā itself turned toward heaven. What looks like it is growing - the compounding balance, the swelling claim - is quietly being erased; and what looks like pure outflow, the sum let go, is the thing being grown. Elsewhere the Qur'an says of the self-interested increase that what you give hoping to gain more through people's wealth "will not increase in God's eyes" - a line the commentators read first about gifts given to get more in return, but whose principle stands over every ledger. The increase you can count is not the increase that is counted.
And behind both ledgers, the verse the earliest Muslims remembered as among the last ever revealed - placed, not by accident, at the close of the passage on debt:
"Beware of a Day when you will be returned to God: every soul will be paid in full for what it has earned, and no one will be wronged."
وَاتَّقُوا يَوْمًا تُرْجَعُونَ فِيهِ إِلَى اللَّهِ ۖ ثُمَّ تُوَفَّىٰ كُلُّ نَفْسٍ مَّا كَسَبَتْ وَهُمْ لَا يُظْلَمُونَ

The passage that begins with a loan ends with a reckoning. Every balance the world leaves open, that Day closes. The ledger the child's finance ministry keeps is not the last one.
The whole hand, not one finger
It is tempting here to find a villain - the banker, the bondholder, the distant institution - and lay the wound at his door. The Prophet ﷺ closed that exit. He is reported to have cursed not one party to the increase but every party to it at once:
"the accepter of interest and its payer, and the one who records it, and the two witnesses," and he said, "They are all equal."
(Ṣaḥīḥ Muslim 1598)
They are all equal. The one who takes it, the one who pays it, the clerk who writes the contract, the witnesses who nod it through - the whole apparatus, not a single face. It does not permit the reader to stand outside the transaction and point. We live inside interest; our pensions rest on it, our savings grow by it, our comfort is underwritten by the very mechanism the child's clinic is starved to serve. The increase is not someone else's sin.
And on the other side, the mercy the increase refuses, the Prophet ﷺ made into the plainest open door. A man from an earlier people, Hudhayfa reports, was met by the angels with no good deed he could summon - except one: I used to lend to people, and I would tell my servants to give the one in hardship more time, and to go easy on him. And God said: We have more right to that than he. Overlook him. And he was forgiven (Ṣaḥīḥ Muslim 1560a). Forgiven for that alone. And the promise made general:
"Whoever would like Allah to shade him with His shade, let him give respite to one in difficulty, or waive repayment of the loan."

Respite, or release. The two things the machine cannot do, and the two things that buy a man the shade of God on the Day there is no other shade.
What this is not
Be careful here, because a verse can be over-read as easily as a scale can be tipped.
This is not a fatwa on the modern financial system, nor a ruling on any bond, any mortgage, any government, any named lender. It is a reflection on what an ancient prohibition still sees. The economists are not wrong that sovereign lending is lawful commerce, that risk deserves a return, that a country which borrows must in justice repay, that a world of pure debt-forgiveness would lend to no one - and the Qur'an itself protects the lender's capital in the same breath it forbids the increase. The point is narrower than abolish debt. It is that an increase which grows because the borrower is weak and while he is weak is not a neutral fact of finance. It is the thing revelation named and refused, and no amount of normalcy launders it into innocence.
Nor is this a claim that a nation's suffering is a punishment. The child without her vaccine is not being disciplined by God for a loan her grandparents' government signed. Debt is a human arrangement, made by human hands - which means it can be unmade by them. That is the hope in it.
Because releasing the debtor has been done. The campaigns that cancelled the debts of the poorest countries a generation ago were not a fantasy; they happened, and children lived who would not have. Several traditions once built debt-release into the calendar itself - a jubilee, a fixed year when the ledgers were wiped and the enslaved-by-owing walked free. What the verse asks of one creditor, whole civilisations have occasionally found the nerve to ask of themselves. It is not utopian. It is periodic. We have simply let this cycle run longer than mercy should allow.
What it costs to weigh it right
So what does the increase ask of you, tonight, far from any finance ministry?
Less than you fear, and more than you would like. It asks, first, that you stop calling the treadmill natural - that when you hear a poor country spends more on interest than on its sick, you refuse the shrug that says that is simply how lending works. It is how this lending works, and it was called something once. It asks that where you hold a debt over someone smaller than you - a person, not a nation - you remember that his lateness is not your opportunity: give him time, and if you can, let it go. And it asks the harder thing of the whole hand the hadith named: that those of us whose comfort rests on the safe and swelling return stop pretending our books are clean, and press, where we have any voice, for the debtor in genuine hardship to be given what the verse commands - not the increase, but the wait, and if mercy can reach that far, the release.
Because the passage that begins with a loan does not end with a loan. It ends with a Day. And between the loan and the Day, one sentence stands open like a hand:
"If the debtor is in hardship, then delay things until matters become easier for him; still, if you were to write it off as an act of charity, that would be better for you, if only you knew."
وَإِن كَانَ ذُو عُسْرَةٍ فَنَظِرَةٌ إِلَىٰ مَيْسَرَةٍ ۚ وَأَن تَصَدَّقُوا خَيْرٌ لَّكُمْ ۖ إِن كُنتُمْ تَعْلَمُونَ

The increase eats. It was always going to eat - the debtor first, and then, on a Day no ledger survives, the one who fed on him.
The mercy is the only thing that grows.
